
I have a client who was instructed to have a “certified notary identity and address certificate” completed and notarized. The requester did not provide the form, or instruct the client where to get one. Does the NNA have access to such a form? — R.A., Michigan
No. Unfortunately, we have never heard of this form. Your client may want to contact the requester to ask where to obtain a copy of the form. We cannot help with what such a form would include, as this would be considered giving legal advice.
