Do you know what a 'Certified Notary Identity' form is?

I have a client who was instructed to have a “certified notary identity and address certificate” completed and notarized. The requester did not provide the form, or instruct the client where to get one. Does the NNA have access to such a form?R.A., Michigan

No. Unfortunately, we have never heard of this form. Your client may want to contact the requester to ask where to obtain a copy of the form. We cannot help with what such a form would include, as this would be considered giving legal advice.

This could've been a scam. But more likely I'm thinking this is the Notary certification of identity form for the client not for the Notary but I have had one where I put my information on there and I will notarize that form as well and that form is usually included in the mortgage when I am doing the documents for a mortgage there will be a notary identity form for the client and sometimes there will be one where I verify the information for myself.

I Google searched the wording "certified notary identity and address certificate" and Google AI has quite an extensive answer describing this "certificate" but cannot show an image of one.

Not long ago, a girl going to study in another country, came in to notarize her Collegue acceptance documents, and on that form also the officials were asking for my Notary Information, Name of Business, Address, Phone #, and email address. I notarized the document and printed my info. two weeks later, a nice woman who identified herself as part of that collegue abroad, called me to verify all the Information I printed, everything checked Okay and she thank me for my time and finished the call.

I believe this is a form that certifies the identity of the person who is requesting to have their identity verified. Not verifying the notary's identity.
